Becoming A Client of The Anabolic Clinic, SC

Charges and Payments
1. Payment is expected at the time of service.
2. Prices are subject to change without notice, unless, a full-year’s payment is made in advance.
3. The Anabolic Clinic, SC, does not accept insurance payment.
4. You agree not to submit Anabolic Clinic, SC, statements/invoices to any third-party for payment or reimbursement.

5. Intake Charges
Call for our current intake charge, which includes:
1) Initial consultation
2) Review of your completed intake questionnaire
3) Development of your Anabolic Clinic sm program
4) Phlebotomy (blood draw)
5) Initial laboratory testing (Routine lab tests are listed below.)
6) History
7) Physical Examination
8) Intake imaging tests
a) Liver screening ultrasound
b) Carotid arteries screening ultrasound
c) Pelvis ultrasound (women only)
d) Testicular ultrasound (men only)
e) Bone densitometry by ultrasound (optional)
9) Body composition testing
10) Body Mass Index

5.a. Optional Intake Charges – men only
a. Semen Analysis (additional fee – required for male contraception clients, optional for others)

5.b. Optional Intake Services (additional fee payable to third party) – men only
a. Sperm Banking (available at any time, not only at intake)

6. Monthly Charges
Call for our current monthly charge, which includes:
1) Monthly office consultation/visit
2) Routine laboratory testing (Performed once every three months. Routine lab tests are listed below.)
3) Phlebotomy (blood draw) for routine laboratory testing
4) Body composition testing (performed once every three months)
5) Follow-up routine physical examination (Exams are performed as needed.)
6) Administration of medications prescribed by us (upon request)
7) Equipment for self-administration of medications we prescribe (e.g., syringes, needles, etc.)

6.a. Additional Charges
1) Routine lab tests performed more frequently than every three months
2) Non-routine lab or diagnostic tests
3) Body composition testing performed more frequently than every three months
4) Follow-up ultrasound examinations of liver, testicles, pelvis, carotid arteries, bone densitometry
5) Telephone consultations
6) Office visits more frequently than monthly – except to administer medication
